Enrollment Forms

The forms on this page are Enrollment Forms necessary for attendance at Rainier High School.  Forms may be downloaded, completed and returned via email to:yanusevichk@rainier.wednet.edu or brought into the Registrar's Office at Rainier High School.  Additional forms are required and will be provided during the enrollment process.  Once we have evaluated the necessary documents, an appointment will be scheduled with the building principal, then the counselor for scheduling.  Parent/guardian attendance at these meetings is required.  For more information, please email: yanusevichk@rainier.wednet.edu or call 360-446-9134 when school is in session.
